In collaboration with the Danish Diabetes Association and Region of Southern Denmark, Steno Diabetes Center has been developing a health education tool kit in the project: ‘Health education targeting vulnerable patients with chronic illness – further development and testing of health education methods and tools.’
The tool kit consists of a booklet (‘Guide to health education tools’) and nine health education tools.
The booklet discusses vulnerability in relation to patient education and describes the conditions and challenges that might be relevant in relation to collaborating with vulnerable patients in the context of patient education.
The booklet provides recommendations for structural initiatives, which could be appropriate to consider when working with this target group. Furthermore, step-by-step guidelines for the health education tools are included.
